Patient Activation Measure®

promoting patient involvement

What is patient activation?

Patient activation refers to a person’s ability to self-care, including the knowledge, skills and confidence to participate in their own health care journey.

The World Health Organisation (WHO) defines self-care as “the ability of individuals, families and communities to promote health, prevent disease, and maintain health and to cope with illness and disability with or without the support of a health-care provider.”

Although patient activation and self-care concepts aren’t new, operationalising these concepts across health systems involves large-scale reform. Successful implementation, especially at a national level, requires a validated, evidence-based tool such as the Patient Activation Measure® (PAM®).

How to measure patient activation

CFEP Surveys is the exclusive Australian licensee of the Patient Activation Measure® (PAM®).

PAM® is a world-renowned assessment tool that has been adopted by leading health systems and organisations, including NHS England and Stanford Health Care, to:

  • Improve segmentation and risk identification among patient populations
  • Tailor individualised care plans
  • Support and educate patients based on PAM® levels, and
  • Measure impact based on activation levels, movement and health outcomes.

By measuring patient activation via the PAM® survey, Australian health services have the ability to:

  • Improve efficiencies with the use of more targeted resources through segmentation
  • Empower and support patients and consumers to increase their self-management abilities
  • Better manage patient populations
  • Drive improved health outcomes and the patient experience
  • Reduce unwarranted utilisation of services and re-admissions
  • Benchmark performance against other service providers, and
  • Lower costs.
